GP list validation – have you changed your details or address?

NHS England is currently carrying out a nation-wide GP list validation (or list cleansing) exercise to identify patients who may have moved away, passed away or not longer live in the area. As part of this process, NHS England or Primary Care Support England (PCSE) may send letters to patients whose details need to be confirmed.
If you receive a letter asking you to verify your details, please respond as soon as possible and do not ignore it.
If no response is received and PCSE is unable to confirm your details, they will notify the practice that you are to be removed form our patient list. We will then need to contact you to check whether you are still living at the registered address.
We have had several cases this week where patients were still eligible to remain registered with the practice but had moved to a new address within our practice area without informing us.
To help keep your records accurate and avoid any disruption to your registration, please let us know promptly if you change your name, address, telephone number or any other personal details.
